1. Patient Records

YEARS: 1966-

VOLUME: 100 Cubic Feet Per Year

ARRANGEMENT: Alphabetical

These are records of dental patients, including registration papers, informed consent sheets, health sheets, payment cards, x-rays, doctors' treatment notes, write-offs, etc..

Authorized Retention:

A. Adult patients: Retain for ten (10) years after all treatment is completed, then destroy as a confidential record.

B. Minor patients: Retain for ten (10) years after patient's 18th birthday, then destroy as a confidential record.

NOTE: Retention times apply to both electronic and paper media.

2. X-Rays

YEARS: 1966-

VOLUME: 45 Cubic Foot Per Year

ARRANGEMENT: Alphabetical

These records are dental x-rays taken of patients.

Authorized Retention:

A. Adult patients: Retain for ten (10) years after all treatment is completed, then destroy as a confidential record.

B. Minor patients: Retain for ten (10) years after patient's 18th birthday, then destroy as a confidential record.

NOTE: Retention times apply to both electronic and paper media.

3. Cytology and Biopsy Reports

YEARS: 1970-

VOLUME: 2 Cubic Feet Per Year

ARRANGEMENT: Alphabetical

These are laboratory cytology and biopsy reports.

Authorized Retention:

 Retain one copy in patient's file. Retain all other copies for period of reference value only and destroy.
